Pediatric Physical Therapist / PTA Homeside Rehab DESCRIPTION Pediatric Physical Therapist or PTA Rochester, NY 14612 Job Summary Homeside Rehab is seeking a compassionate and skilled Physical Therapist or PTA to join our pediatric home health team. In this role, you will deliver individualized, evidence-based therapy to pediatric patients in their homes, making a meaningful impact while enjoying schedule flexibility and a supportive team environment. Job Responsibilities Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements Collaborate with the clinical team to ensure coordinated and effective treatment Communicate clearly with office staff, patients, and caregivers regarding progress and care plans Job Qualifications ActiveNY Physical Therapy License (PTA) — required Basic computer proficiency for electronic documentation Strong clinical assessment and interpersonal communication skills Ability to work independently in a home health environment Valid NY driver's license and reliable transportation for home visits Pediatric experience is preferred Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings Job Details Job Type: Full-time, Part-time, Per Diem Location: Rochester, NY 14612 Salary: $65 - $80 per hour Schedule Flexibility: Choose your own hours, day and evening shifts available, Monday to Friday, with weekend availability as needed Benefits Flexible schedule Referral bonus Therapist of the Month bonus License/Certification New York State Physical Therapy License (Required) For questions, contact Jan at View email address on click.appcast.io or View phone number on click.appcast.io. #J-18808-Ljbffr Homeside Rehab
