Work Environment\n

This full-time position is set in a primarily remote work environment, with occasional visits to local schools in Cleveland, Tennessee. While the flexibility of remote work fosters a balance between professional and personal life, it also requires adaptability to meet the urgent needs of our clients. The typical work schedule will be Monday to Friday, 8:00 AM to 4:30 PM, with the possibility of evening or weekend hours when necessary to ensure continued support for seniors and their families. On-call availability may also be required for urgent situations.

Key Responsibilities\n

• Conduct detailed psychosocial assessments to evaluate the emotional and mental health needs of senior clients and their families.

• Collaborate with clients, caregivers, and interdisciplinary teams to create customized care plans that meet individual goals, preferences, and challenges.

• Provide expert counseling and emotional support, assisting clients in navigating transitions such as moving into assisted living or dealing with the loss of a spouse.
\n
• Advocate for clients’ rights and access to essential community resources including healthcare, housing, and social services, ensuring no barriers to needed support.
\n
• Regularly monitor client progress through home visits or virtual check-ins, addressing any safety concerns or changes in living conditions.

• Facilitate family communications to resolve conflicts and enhance the support network for the senior clients.
\n
• Educate clients and families about community programs, support groups, and social engagement opportunities, connecting them with the resources that can dramatically improve their quality of life.

• Maintain thorough, accurate documentation including client assessments, care plans, and progress notes in accordance with organizational policies.

• Stay abreast of current best practices and compliance regulations within the field of social work.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

Compensation and Benefits\n
