Quick Facts Location: 1700 Tuttle Street, Baraboo, WI 53913 Start Date: 10/05/2026 End Date: 04/05/2027 Shift: Day shift, 8:30 AM – 4:30 PM or 9:00 AM – 5:00 PM (flexible; no call) Hours/Week: 40+ hours Employment Type: Contract (26 weeks) License/Certs Required: IMLC license, WI license, BLS certification Experience: Prior primary care experience; geri-focused and managing patients with multiple comorbidities preferred Pay & Benefits Hourly Rate: $148.98 Pay Package: Competitive pay discussed upon application Benefits: Weekly pay via direct deposit Comprehensive medical benefits (W-2) Paid sick time in accordance with applicable laws Licensure, certification, travel, and other reimbursements when applicable Robust referral bonus program 24/7 dedicated support team during assignment What You’ll Do Provide primary care services to a diverse patient population across all ages, with an emphasis on geriatrics and chronic conditions such as diabetes and heart disease Support a 1:1 clinical team including MA, LPN, or RN staff Manage an average of 18-22 patients per day, ensuring quality care and patient satisfaction Perform standard primary care procedures such as physicals, chronic disease management, medication management, and health screenings Document patient encounters accurately using Epic EMR system Collaborate with clinical team and support staff to deliver seamless patient care Participate in patient triage during business hours, supporting the doctor of the day as needed What You Need Required: MD or DO license with IMLC/ Wisconsin licensure Board certification in Family Medicine or Internal Medicine BLS certification Preferred: ACLS certification Experience with geriatric populations and managing multiple comorbidities About the Assignment This primary care clinic serves patients of all ages, with a focus on chronic illness management, especially in the elderly population. Providers will deliver holistic, patient-centered care in a well-equipped environment using Epic EMR.
