We are seeking a Registered Nurse Case Manager to coordinate comprehensive care, support appropriate utilization, improve patient flow, and facilitate safe, timely discharge. This role offers a base pay range of $31.94–$43.92 per hour, stackable evening and weekend differentials, benefits, and a $10,000 sign-on bonus with a required two-year commitment.

Responsibilities

• Perform medical and psychosocial assessments.
• Create and manage discharge plans.
• Coordinate care with physicians, nurses, social workers, and other team members.
• Lead care planning discussions.
• Review medical necessity and admission status.
• Ensure the appropriate level of care, including inpatient versus observation status.
• Communicate with insurance companies and payors.
• Escalate authorization and status issues.
• Identify discharge barriers early and improve patient flow and length of stay.
• Arrange post-discharge services, including rehabilitation and home care.
• Follow regulatory and hospital guidelines.
• Maintain accurate documentation and report safety or quality concerns.

Required Qualifications

• Either 3 or more years of recent acute-care case management experience, or 3 or more years of RN experience in Med/Surg, Telemetry, ICU/PCU, ED, or Neuro.
• Home health or insurance case management candidates must have 3 years of total acute-care experience, including 1 year of acute-care experience within the last 5 years.
• Active Florida RN license or compact license.

Preferred Qualifications

• Experience coordinating care, managing utilization, facilitating discharge, and communicating with payors in an acute-care setting.

Compensation and Benefits

• Base pay: $31.94–$43.92 per hour.
• Evening differential: +$2.50 per hour.
• Weekend differential: +$2.00 per hour.
• Maximum combined shift differential: +$4.50 per hour.
• $10,000 sign-on bonus with a two-year commitment.
