Housing Case Manager

Overview Housing Case Manager The primary responsibility of this role is to deliver comprehensive care and support to enhance the well-being of service recipients. This involves providing therapeutic support and skills training in person and virtually, fostering communication with clinical professionals and the individual's support system as authorized. The position acts as a role model and mentor, coaching service recipients using approved techniques and strategies and implementing physical interventions when necessary to ensure safety. Additionally, meticulous completion of documentation and reporting for grant programs is essential to contribute to the team\'s operational effectiveness. Responsibilities Deliver comprehensive care and support to enhance service recipients\' well-being Provide therapeutic support and skills training in person and virtually Foster communication with clinical professionals and the individual\'s support system as authorized Act as a role model and mentor, coaching service recipients using approved techniques and strategies Implement physical intervention when necessary to ensure safety Complete documentation and reporting for grant programs accurately and timely Minimum Requirements Required Education : Bachelor\'s degree in health or human services Acceptable Degrees: Applied Behavior Analysis, Child Development, Counseling, Early Childhood Education, Early Childhood Intervention, Education Psychology, Gerontology, Human Growth and Development, Medicine, Nursing, Physician Assistant, Psychology, Registered Nurse, Rehabilitation & Rehab Studies, Social Work, Sociology, and Special Education Substitutions OR bachelor\'s degree with at least 30 hours of behavioral health coursework Licenses Required License(s) Valid driver\'s license with an acceptable driving record Eligible to obtain Qualified Mental Health Professional (QMHP-CS) certification Must provide license (if applicable) and official documentation of all educational attainment (Transcript/Degree/Certificate) Application Note For a complete Job Description or to reach out with questions, contact View email address on click.appcast.io #J-18808-Ljbffr
