If so, apply today!As a Registered Nurse Case Manager, you'll contribute to our success in the following ways:Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.What is Required?Current State license as a Registered Nurse.Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing1+ year of experience as a nurse in a clinical care setting.Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipmentMust possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.Experience in hospice or home health environment is preferredWhat's in it for you?Comprehensive insurance plans for medical, dental, and vision benefits including pet insurance401(K) with employer matchTuition reimbursement for full-time staff and continuing education opportunities for all employees at no costPaid time off, paid holidays, family, and pet bereavementFleet vehicle program (restrictions apply) or mileage reimbursement.* Benefit eligibility can vary by position and shift status.Equal Employment Opportunity: We are proud to be an equal opportunity workplace and comply with state and federal affirmative action requirements.
