Case Management Coordinator

Randstad Healthcare is hiring for a Medical Case Manager in Homestead/Florida City (33033, 33032). Candidates must reside in this area. We are seeking a self‑motivated, energetic, detail‑oriented, highly organized, tech‑savvy Case Management Coordinator to join our team. Our organization promotes autonomy through a Monday–Friday (8:00 AM–5:00 PM) schedule and flexibility as you coordinate the care of members. The coordinator is responsible for telephonic and/or face‑to‑face assessment, planning, implementation, and coordination of all case‑management activities to evaluate member medical needs and facilitate overall wellness. Cases may include supportive and medically complex members, requiring proactive action to address issues and enhance short‑ and long‑term outcomes, including integration of services and supports such as prior authorizations, coordination with PCPs and skilled providers, condition management, medication review, and community resources. Role requires 50‑75 % travel for face‑to‑face visits with members in Miami Dade County, FL. Candidates must reside in Homestead/Florida City (33033, 33032). Bilingual (Spanish, English) fluently speaking and writing. Ability to multitask, prioritize, and adapt to a fast‑paced changing environment. Effective verbal and written communication skills. Travel approximately 75 % of the time within Miami‑Dade County to see members in homes, assisted‑living facilities, and skilled nursing facilities. Must be fluent in English & Spanish and able to hold a full conversation.
