Hospice RN Team Lead: Care Coordination & Mentorship
Advocate Health Care
Advocate Health Care seeks a Registered Nurse (RN) to provide direct patient care and coordinate outcomes across multidisciplinary teams. The role includes patient assessment, care planning, and documentation in accordance with policy. The RN will educate staff, participate in discharge planning, and ensure timely communication with physicians and families.
