Hospice RN Case Manager — Compassionate Home Care Leader

Gentiva is seeking a Hospice RN / RN Case Manager to lead patient care in home settings across our hospice network. You will perform comprehensive RN assessments, develop and update plans of care, and coordinate interdisciplinary team with physicians, social workers, and chaplains. This role emphasizes compassionate, patient-centered care and effective education for families.
