SOCIAL SERVICE DIRECTOR ASHBROOK CARE & REHABILITATION CENTER, Scotch Plains, NJ, US RESPONSIBILITIES AND ACCOUNTABILITIES Plans and directs a comprehensive social services program which provides counseling and other support services for residents and families. Consults with the Administrator and other department heads regarding interdisciplinary issues. Works directly with residents and families experiencing personal and environmental difficulties or concerns related to their physical or emotional condition. Assures that medically related social services are provided to maintain or improve each resident’s ability to control everyday physical needs (e.g., appropriate adaptive equipment for eating, ambulating, etc.) and mental and psychosocial needs. Maintains a written record of the frequency and nature of the social service consultation and services provided or obtained. Performs an evaluation of each resident’s social needs; formulates and records a care plan in the resident’s medical record and periodically re-evaluates in conjunction with the total plan of care. Collects pertinent social data upon admission of each resident to be placed in the medical record, including information about personal and family problems related to the resident’s illness and care, and actions taken to meet the resident’s needs; makes this data available to the attending physician and other appropriate staff members. Assists each resident with adjustment to the social and emotional aspects of placement by including clearly defined social service goals and interventions in the care plan. Coordinates the interdisciplinary care plan team, schedules conferences in a timely manner, and invites residents and family members as appropriate. Acts as a liaison between residents, families and outside agencies, and the facility Administrator, to ensure that residents’ rights are maintained. Assumes the roles and responsibilities of the Admissions department in the absence of the Admissions/Marketing Director. Functions in a management capacity and adheres to all policies and procedures of the facility as a representative of the care center’s administration. Performs administrative requirements, such as completing necessary forms and reports, under the direction of the Administrator, and submitting such reports to the Administrator. Coordinates discharge planning, including the development of an organized discharge plan for all residents. Ensures the safety of all residents by minimizing the potential for fire and accidents; ensures the facility adheres to legal, safety, health, fire, and sanitation codes, including familiarity with MSDS. Ensures residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes individual needs and rights. Reports occupational exposures to blood, bodily fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy. Reports work‑related injuries and illnesses immediately to the supervisor. Completes all assigned training and skill competency as a condition of employment. Participates in all life‑safety and emergency drills and trainings. Fulfills responsibilities during implementation or activation of the facility’s emergency plan. Assists with evacuation of residents in an emergency situation. Maintains confidentiality of Protected Health Information, including verbal, written, and electronic communications. Reports non‑compliance with policies, procedures, regulations or breaches in confidentiality to appropriate personnel. Protects residents from abuse and cooperates with all investigations. Reports any allegations of abuse, neglect, misappropriation of property, exploitation or mistreatment of residents to the supervisor and/or administrator. Reports any retaliation or discrimination to the Human Resource Director and the Compliance Officer. Reports any suspicion of a crime to law enforcement and the state agency. #J-18808-Ljbffr
