In this role, you'll have the opportunity to make a meaningful impact by helping patients regain independence and improve their quality of life, all from the comfort of their homes or where they reside.If you're someone who values autonomy, flexibility, and building strong patient relationships, this is a great opportunity to thrive in a supportive, collaborative environment.What You'll DoProvide in-home physical therapy services, including evaluations, start of care visits, follow-ups, recertifications, OASIS assessments, and dischargesDevelop and implement personalized treatment plans based on patient needs and physician ordersGuide patients through therapeutic exercises, gait training, balance and coordination activities, and mobility improvementEducate patients and caregivers on safety, fall prevention, and daily living strategiesMonitor patient progress and adjust treatment plans to achieve optimal outcomesAssess vital signs as needed and communicate any concerns to the care teamRecommend and educate on the use of assistive devices and durable medical equipment (DME)Collaborate with an interdisciplinary team including nurses, occupational therapists, speech therapists, and physiciansComplete timely and accurate documentationSupport discharge planning and ensure continuity of careWhat We're Looking ForLicensed Physical Therapist in the state of MassachusettsDegree from an accredited Physical Therapy programCPR certification (current)Valid driver's license and reliable transportationHome health experience requiredStrong communication, organization, and clinical decision-making skillsPreferredAt least 1 year of clinical experienceWhy Join Us?Autonomy and flexibility in your daily scheduleMeaningful, one-on-one patient careSupportive and collaborative team environmentOpportunities for professional growth and continuing educationWork Environment & Physical RequirementsThis role involves travel to patient homes and requires physical activity including standing, walking, bending, and occasional lifting or assisting patients.
