Care Coordinator

To support efficient client service management, the full-time remote Care Coordinator will handle the intake, triage, and validation of incoming client service requests from primary care providers, ensuring compliance with internal standards and timely routing to the clinical review team. Key responsibilities Monitor queues for new eConsult referrals and initiate the intake process Accurately complete mandatory non-clinical intake fields and gather necessary supporting documentation Track eConsults and escalate any issues requiring additional information to ensure timely service delivery Required qualifications High School Diploma or equivalent required 1+ years of experience in a healthcare administrative or clinical support setting Experience with multiple Electronic Health Records (EHRs) strongly preferred Medical Assistant (MA) or equivalent administrative/clinical background preferred Experience with multispecialty referral coordination or utilization management preferred
