Key Responsibilities

• Conduct comprehensive evaluations of patients' physical capabilities, analyzing their abilities and challenges to establish effective, targeted treatment plans.
• Administer therapeutic interventions in patients' homes, utilizing diverse treatment methods such as manual therapy, therapeutic exercises, and modalities to alleviate pain and enhance mobility.
• Develop customized treatment plans that align with patients' functional goals, ensuring they achieve maximum potential in their everyday activities.
• Regularly document treatment sessions, including patient progress and responses, to ensure accurate records and continuous improvement of care.
• Reassess patient conditions and modify treatment plans accordingly to facilitate optimal recovery and adapt to changing needs.
• Advise patients and their families on the use of assistive devices, ensuring they are well-informed about how to support self-care and mobility.
• Provide educational sessions for patients, caregivers, and other healthcare team members, fostering a collaborative approach to physical therapy.
• Maintain current knowledge of best practices by participating in ongoing educational opportunities and staying engaged with professional organizations.

Patient Population / Client Focus

Qualifications

• A Doctorate Degree in Physical Therapy is required.
