We serve with compassion and integrity, transforming lives within our communities.Position PurposeThe Registered Nurse (RN) – Home Care is responsible for delivering skilled nursing care to PACE participants in their homes, ensuring comprehensive, coordinated, and compassionate care. This role collaborates closely with the interdisciplinary team to assess, plan, and implement care that supports participants' independence and overall well-being.What You'll Do:Provide skilled nursing care to participants primarily in their homes, ensuring safety, comfort, and quality outcomesConduct comprehensive assessments and develop individualized care plans in collaboration with the interdisciplinary teamMonitor participant health status and respond promptly to changes in conditionAdminister medications, treatments, and interventions as orderedEducate participants and caregivers on disease management, medications, and care plansCoordinate care with physicians, therapists, and other care team membersDocument care in accordance with regulatory and organizational standardsParticipate in interdisciplinary team meetings and contribute to care planning discussionsEnsure timely communication of participant needs, concerns, and changes in conditionMinimum Qualifications:Graduate of an accredited School of Nursing; Associate or Bachelor's degree requiredCurrent Registered Nurse (RN) license in the state of LouisianaMinimum of one (1) year of nursing experience required; experience in home care, geriatrics, or PACE preferredValid driver's license with reliable transportation and a clean driving recordAbility to work a flexible schedule, including occasional weekendsPosition Highlights and Benefits:Comprehensive benefits including medical, dental, vision, and paid time off from day one.Access to Daily Pay for earned wages.Supportive, patient-centered environment.Extensive orientation and ongoing professional development opportunities.Our CommitmentRooted in our Mission and Core Values, we honor the dignity of every person and recognize the unique perspectives, experiences, and talents each colleague brings.
