Work Environment\n

This role is primarily performed in a remote setting, which allows for flexibility in managing your schedule. You will connect with clients primarily via telehealth services but may also need to conduct in-person visits to community facilities, clients' homes, or designated healthcare settings as required. We operate Monday through Friday, with core hours from 8:00 AM to 4:30 PM, but occasional evening or weekend hours may be necessary to accommodate client needs.

Key Responsibilities\n

• Conduct thorough assessments of each client's psychosocial needs, focusing on emotional, mental, and social factors influencing their overall health.

• Collaborate closely with clients and their families to create tailored care plans that align with individual goals, preferences, and specific circumstances.

• Deliver counseling sessions to provide emotional support for navigating transitions such as loss, health changes, or caregivers' challenges.

• Advocate for clients' rights and facilitate access to community resources like support groups, financial assistance programs, and healthcare services.

• Engage with interdisciplinary teams, including medical professionals, therapists, and caregivers, to ensure holistic care and support for each client's wellbeing.

• Conduct regular follow-up visits to monitor client status, reassess needs, and adjust care plans accordingly.

• Educate clients and families on available resources and assist in building connections within the community to reduce social isolation.

• Maintain meticulous documentation in compliance with ethical and legal standards, recording assessments, care plans, and follow-up notes.

• Stay current with social work practices and continue professional development through training and education opportunities.

Patient Population / Client Focus\n

This position primarily serves the senior population in the Waco area, many of whom face unique challenges related to aging, mental health, and social isolation. You will work closely with older adults and their families, advocating for their needs and ensuring they have access to necessary resources. By providing emotional support and practical guidance, you will play a crucial role in enhancing clients' quality of life and promoting their independence.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
