Key Responsibilities\n

As a Licensed Clinical Social Worker at Guideline Healthcare, you will:

• Evaluate, diagnose, and treat a diverse range of psychosocial challenges faced by patients and their families, employing your clinical expertise to assist in overcoming personal and environmental barriers.

• Complete thorough bio-psychosocial assessments to identify and eliminate obstacles to treatment while enhancing the overall health status of our clients.
\n
• Consult with medical and support staff to create comprehensive care plans tailored to each patient’s needs, facilitating improved treatment compliance and optimal wellness.

• Develop and implement individualized treatment plans, utilizing both individual and group therapy modalities, adjusting interventions as necessary to maximize health outcomes.

• Document all social work activities and patient interactions in compliance with departmental standards, ensuring proper tracking of progress and outcomes.
\n
• Provide educational support to patients and their families to help them understand medical and social work recommendations aimed at restoring the patient's functional capacity.

• Actively engage in professional development activities to enhance your expertise in relevant clinical areas, including attending training sessions specific to your role.

• Adhere to the NASW Code of Ethics and all departmental policies and procedures to maintain a safe and compliant care environment.

Patient Population / Client Focus\n

Your primary focus will be on outpatient clients at our Detox Center, where you will work with individuals facing mental health challenges, including those navigating substance use disorders. Your role will involve advocating for patients' and families' interests, ensuring access to social support systems and community resources, and enhancing their overall psychosocial functioning.

Qualifications\n

To be considered for this vital role, candidates must possess:

• A Master’s Degree in Social Work from an accredited program.

• A current LCSW license issued by the State Board of Social Work Licensure.

Preferred Experience\n
