Key Responsibilities\n

• Conduct thorough psychosocial assessments to gather vital information about clients' mental health concerns, strengths, and available resources.

• Provide individual, family, and group therapy sessions, utilizing a range of evidence-based treatment modalities such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care.

• Develop and implement personalized treatment plans with measurable goals and interventions, ensuring they align with the clients' needs.

• Collaborate with psychiatrists, psychologists, and other healthcare providers in a multi-disciplinary team to coordinate and enhance client care.

• Facilitate crisis intervention strategies, risk assessments, and safety planning for clients in acute distress.
\n
• Maintain meticulous documentation of client interactions and treatment plans in accordance with ethical and legal standards.

• Lead group therapy sessions, psychoeducation classes, and support groups to foster community and shared experiences among clients.

• Stay current with best practices in mental health treatment and contribute to a culture of continuous improvement and learning within the organization.

Patient Population / Client Focus\n

In this role, you will serve a diverse clientele, primarily within a hospice setting. Your work will focus on supporting individuals facing end-of-life issues, grief, and loss, along with their families. You will provide tailored therapeutic support that addresses their emotional and psychological needs, helping them navigate these challenging circumstances with dignity and compassion.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
