Details Home health setting providing physical therapy services to patients in their homes Patients seen within a 45-mile radius of the Albuquerque area, including Tijeras, Rio Rancho, Bernalillo, Bosque Farms, Los Lunas, and Belen Monday through Friday schedule 13-week assignment Requirements Active PT license in the state of practice required (license required if applicable) Graduate of an accredited physical therapist program recognized by CAPTE, APTA, or CAHEA Current CPR certification required 2 years of clinical experience preferred; home health experience a plus but not required Reliable transportation, valid driver's license, and auto insurance required for home visits Basic computer proficiency with willingness to learn EMR and OASIS documentation systems Additional Information Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications per Medicare and agency standards Develop and implement individualized plans of care aligned with physician orders Supervise Physical Therapist Assistants and Home Health Aides in compliance with state practice acts Consult with physicians and interdisciplinary team members on treatment adjustments and patient progress Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies Complete OASIS assessments, recertifications, transfers, resumptions of care, and discharges within agency timeframes Apply ICD-10 coding for accurate documentation Participate in QAPI activities and periodic staff in-service programs Work independently with minimal daily oversight in varied home care settings Maintain compliance with HIPAA and infection control, safety, and risk prevention protocols American Traveler Job ID P-750666.
