Clinical Case Manager: Adoption Support & Preservation

Brightpoint in Chicago is seeking a Clinical Case Manager for Adoption Support and Preservation (ASAP). You will assess behavioral health needs, develop treatment plans, and deliver or coordinate therapeutic interventions in home, community, or office settings. This full-time, hybrid role requires a Master’s degree or a Bachelor’s degree with relevant experience, plus a valid driver’s license.
