RN/LPN Wellness Nurse

Maplewood Brewster LLC, a senior living community offering assisted living and memory care services in Brewster, MA, is seeking a compassionate and dedicated RN/LPN Wellness Nurse to join our Healthcare & Medical Services team. In this role, you will provide resident-centered clinical care, helping to enhance the quality of life for older adults in a supportive and engaging community. As an RN/LPN Wellness Nurse, you will be responsible for ongoing wellness assessments, development and implementation of individualized care plans, and coordination of care with physicians, families, and the interdisciplinary team. You will administer medications and treatments, monitor residents’ conditions, and promptly respond to changes in status, ensuring that each resident receives safe, evidence-based, and respectful care. Your clinical judgment and attention to detail will support early identification of health concerns, fall and infection prevention, and overall wellness promotion. You will play an active role in supporting residents living with memory loss, using person-centered approaches to manage behaviors, maintain dignity, and foster meaningful engagement. Daily responsibilities will include accurate and timely documentation in electronic medical records, communication of updates during shift reports, and collaboration with wellness, activities, and dining teams to support each resident’s physical, emotional, and social needs.
