Role: Registered Nurse (RN) Case Manager PRN Coordinate comprehensive, patient-centered care by developing and managing care plans that align with patients’ health needs and support cost-effective treatment. Key Responsibilities Assess patient needs and coordinate care across the continuum. Develop, implement, and revise individualized care plans. Coordinate services and resources to ensure appropriate, timely interventions. Support safe, high-quality care delivery and continuity of care. Communicate with interdisciplinary teams and document care plan activities. Help optimize care outcomes while promoting cost-effective options. Qualifications & Skills Current RN licensure. Strong assessment and clinical decision-making skills. Excellent communication and collaboration with interdisciplinary teams. Ability to manage complex cases and coordinate multiple care needs.
