Bilingual Spanish Field Registered Nurse – Home Care

Brooklyn, NY – Bushwick

\n

Monday–Friday | 35 hours/week

\n

Field-based | 5 visits per day

\n

️ Spanish bilingual required

A Brooklyn-based home care organization is looking for a bilingual Spanish Registered Nurse to join its team. This is a field-based role focused on assessing clients, coordinating care, developing and monitoring plans of care, and supporting patients and their caregivers in the home.

\n

What you’ll do:

\n

\n
• Complete comprehensive in-home nursing assessments and reassessments
\n
• Develop, implement, and monitor individualized plans of care
\n
• Coordinate services with clients, families, physicians, care providers, and community partners
\n
• Provide ongoing patient/caregiver education and support
\n
• Make recommendations regarding continuation or modification of home care services
\n
• Supervise Home Health Aides and assign duties based on client needs
\n
• Respond to changes in condition and emergencies as needed
\n
• Maintain accurate and timely clinical documentation
\n
• Coordinate with internal and external healthcare teams to ensure continuity of care
\n
• Participate in quality improvement initiatives and staff meetings
\n

\n

What they’re looking for:

\n

\n
• Current New York State RN license
\n
• Bilingual Spanish required
\n
• Home care or CHHA experience strongly preferred
\n
• 2+ years of home health experience preferred
\n
• 3+ years of relevant nursing experience
\n
• BLS certification
\n
• Strong assessment, organization, communication, and care coordination skills
\n
• Comfortable working independently and traveling to clients’ homes throughout NYC
\n
