Baptist Home Health Care by BAYADA is looking for a compassionate and dedicated Full Time Physical Therapist to join our team in Orange Park, FL . This office serves adult and geriatric clients on a per visit basis. Baptist Home Health by BAYADA offers full‑time schedules. Responsibilities Make home visits to clients in designated geographic territories. Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, ambulation patterns, strength and endurance of muscles, balance testing, and safety assessments. Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence‑based dressings and topical agents. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions. Continuously assess and revise the Physical Therapy care plan, and collaborate with nursing in multidisciplinary care planning as appropriate. Educate and instruct clients, family members, or other representatives in rehabilitative care and activities necessary to promote client health, safety, and independent living. Accurately document observations, interventions, and evaluations related to client care using a touch‑pad tablet. Qualifications Current Florida Physical Therapist license. Minimum one year of recent work experience as a Physical Therapist. Graduated from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem‑solving skills. Solid computer skills; prior experience with electronic medical records (EMR) preferred.
