Job Summary Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing role in Saginaw, Michigan. This is a full-time position, 40 hours per week with an 8‑hour shift schedule. The role is ongoing and offers a competitive pay per visit compensation model, a 4‑month orientation pay guarantee, and an opportunity to earn $20,000 in total bonus potential, including a $10,000 first‑year success bonus and a $10,000 sign‑on bonus. Responsibilities Provide management and delivery of patient care plans as a home health registered nurse case manager. Lead and collaborate with the care team to ensure timely, high‑quality patient care. Travel to patient locations, spending approximately 20% of your day driving. Maintain accurate and timely charting to meet critical deadlines. Use tablets, mobile phones, and EMR software to document and manage care. Participate in a 24/7/365 after‑hours care team. Qualifications Active RN license in the state of employment. Graduate from an approved school of practical nursing. Minimum one year of RN experience with clinical competence in patient care. Current driver’s license and willingness to travel regularly. Strong time‑management and self‑management skills. Experience with tablets, mobile phones, and EMR software.
