Work Environment

This full-time position is based onsite, primarily in the homes of our patients within the Cherry Hill area. The role involves a dynamic work schedule of 40 hours per week with some variable weekends, promoting work-life balance while providing essential rehabilitation services. Key Responsibilities

• Conduct comprehensive examinations and evaluations of patients, assessing their functional limitations and disabilities in a home environment.
• Develop tailored treatment plans that address individual patient needs, ensuring effective rehabilitation and recovery processes.
• Consult with healthcare providers about patients' continuum of care and any modifications needed for their treatment plans.
• Deliver high-quality care by implementing and monitoring therapy interventions, making adjustments based on patient progress and feedback.
• Document evaluations, treatment plans, and discharge summaries accurately, adhering to all applicable laws and professional standards.
• Engage with patients and their families, educating them about treatment processes and empowering them to participate actively in their rehabilitation journey.
• Collaborate with interdisciplinary team members, including physicians and healthcare assistants, to ensure a holistic approach to patient care.
• Participate in professional development activities, including continuing education programs, to enhance your skills and knowledge.

Patient Population / Client Focus

The Physical Therapist will work with diverse patient populations, including elderly individuals, those recovering from surgery, and patients managing chronic conditions such as arthritis and neurological disorders. The focus is on tailoring treatments to the specific needs and cultural backgrounds of each patient, ensuring respectful and effective rehabilitation efforts.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• Valid State License in Physical Therapy is mandatory to practice within New Jersey.
• Basic Life Support (BLS) certification is required to ensure patient safety.
• Excellent written and verbal communication skills are essential for patient education and collaboration with team members.
• Proficiency in word processing and electronic health record systems is necessary for documentation and communication.

Preferred Experience

Compensation and Benefits
