Work Environment

The Physical Therapist role operates onsite within patients' homes, providing a comfortable and familiar setting for therapy. This position requires a high degree of flexibility, as you will work directly with clients in their unique environments. The hours for this role are Monday to Friday from 8 AM to 3 PM, offering a structured yet supportive schedule conducive to work-life balance.

Key Responsibilities

• Conduct thorough Physical Therapy evaluations for a variety of patients, assessing their mobility, strength, and overall functionality.
• Develop, implement, and adjust individualized treatment plans based on clinical findings, patient goals, and progress updates.
• Regularly reassess clinical signs and symptoms to determine treatment efficacy, modifying goals and methods as needed.
• Document all patient interactions meticulously within the electronic record system, including SOAP notes, progress summaries, and care plans to ensure continuity of care.
• Educate patients and their families about the recovery process, empowering them to participate actively in their healing journey.
• Achieve minimum utilization targets while contributing to overall business objectives, ensuring seamless operations within the home care setting.
• Collaborate effectively with multidisciplinary peers to provide comprehensive care for patients receiving multiple therapy services.

Qualifications

• A Master's degree or Doctorate in Physical Therapy from an accredited program is required.
• Current and unrestricted licensure to practice physical therapy in the state of New Jersey.
