RN Hospice Business Development Care Planner – Bronx, NY Location: The Bronx, NY, US Salary: $115,000 - $131,500 per year Job Description Working on-site at a major medical center in the Bronx, NY. Work schedule is Monday through Friday, 9 am to 5 pm (no weekends). Responsibilities The RN Hospice Business Development Care Planner is responsible for evaluating and establishing a plan of care for patients referred for hospice services. The RN facilitates admission of referrals upon discharge from the hospital to a hospice program that meets the needs of the patient and family members. As a key member of the business development and marketing team, the Hospice Care Planner develops and maintains positive relationships with physicians, social workers, case managers and discharge planners within the medical center. Qualifications Mid level experience. RN license required. Experience in hospice or related care planning preferred.
