SCAN Health Plan in Oceanside, CA, is seeking a Community Health Worker to support frail seniors in aging in place. You will work with members telephonically and in person, implement plans of care, and advocate for individualized health goals while coordinating with a multidisciplinary team. Must have a High School Diploma and a Community Health Worker Certificate.
