RN - Case Manager
Unit Details: Ability to assess, evaluate and drive discharge plan. Coordinate services to the patient's appropriate level of care. Ensure that treatment plan is initiated, evaluated and re-evaluated daily. Follow the progress and lack thereof daily. Monitor length of stay and identify barriers to discharge. Follow Medicare regulatory guidelines regarding patient choice, IMM notices and MOONS.
Schedule: Monday-Friday, Weekend: None, Call: None, Floating: Within facility/scope of practice as needed
Minimum Experience: 3 years - Unit does not accept first time travelers
Clinical Screen: Candidate will be reviewed by clinical to determine clinical fit, if clinical pass and within Piedmont guidelines, candidate will be sent to the manager for final decision. Clinical screens will be sent on case-by-case basis.
RTO: Max 7 days
Holidays: If one major holiday falls in his/her assignment period, the traveler is expected to work it. If two major holidays fall in his/her assignment period, the traveler is expected to work both of the holidays. If three major holidays fall in his/her assignment period, the traveler is expected to work two of the holidays. Major Holidays are listed below: Memorial Day, Independence Day, Labor Day, Thanksgiving/Black Friday, Christmas Eve/Christmas Day, New Years Eve/New Years Day
Compliance: Full compliance required to start on time; if not compliant in time the push back is 2 weeks due to bi-weekly orientation schedule. Locals accepted - same rate
