Case Manager Substance Abuse

Position Summary The Substance Use Case Manager will provide comprehensive case management services to individuals with substance use disorders, working evening expanded hours. The role focuses on assessing client needs, developing individualized service plans, coordinating treatment and supportservices, and promoting long‐term recovery. The case manager collaborates with the senior counselor, clients, families, treatment providers, and community resources to ensure continuity of care and uphold harm‐reduction, recovery‐oriented goals. Qualifications • Bachelor's degree in a related field required. • CADC preferred; valid driver's license required. • Annual American Heart Association BLS and Heart Saver First Aid, and CPI certification required within 90 days of hire or transfer. • Ability to pass a background check through fingerprint process and drug screen as required. • Prior case‐management experience required. Performance Expectations The case manager will work closely with clinical staff, peer support specialists, healthcare providers, courts, probation, and community partners. They will participate in multidisciplinary team meetings and case conferencing, advocate for client needs across the system, and help reduce barriers to treatment. Duties and Responsibilities Conduct initial screenings and ongoing needs assessments. Develop, implement, and monitor individualized treatment and recovery service plans. Provide crisis intervention, individual counseling, and strength‐based support. Facilitate referrals for mental health, housing, employment, court, and other supportive services. Monitor client progress, provide follow‐up, and adjust service plans as needed. Coordinate or provide transportation for clients when required. Maintain accurate, timely, confidential documentation in compliance with HIPAA, state regulations, and AtlantiCare addiction services policies. Ensure compliance with grant requirements, program standards, and funding guidelines. Skills Strong interpersonal and communication skills. Ability to build rapport with clients and maintain professional boundaries. Ability to problem‐solve and manage crisis situations. Organizational skills and attention to detail. Ability to work with a diverse population. Can work independently and as part of a team. Work Environment This position requires work in office, community, and field settings, with occasional evening or weekend hours. The role involves repetitive motion, hearing, pushing, reaching, reading, speaking, standing, and walking, as well as use of a computer terminal, fax machine, copier, and telephone. Occasional driving is also required. Reporting Relationship The position reports to department leadership.
