Key Responsibilities\n

• Conduct thorough biopsychosocial assessments tailored to each individual’s mental health, substance use, and medical needs in a primary care context.

• Provide direct behavioral health services, including individual and group counseling, intake evaluations, treatment planning, and crisis intervention. Your assessments and treatment strategies will guide patients toward effective recovery.

• Maintain accurate electronic documentation of mental health evaluations, progress notes, and treatment plans, ensuring compliance with applicable regulatory standards.

• Act as a liaison for patients by connecting them to local resources, community agencies, and medication assistance programs, enhancing their access to continued support.

• Collaborate with Quality Improvement teams to establish and refine processes for data collection, ultimately driving better patient outcomes through evidence-based practices.

Patient Population / Client Focus\n

You will work primarily with individuals, couples, and families affected by addiction and mental health disorders. Qualifications\n

• A Master’s degree in Social Work from an accredited institution

• A valid state license as a Licensed Clinical Social Worker (LCSW) is required

• Three (3) years of post-licensure experience in a clinical or mental health setting
