Essential daily duties include:

• Provide primary medical care and palliative care for chronic and acute illnesses to patients who are home-bound or have difficulty traveling to a physician's office
• Provide physical exams, diagnose and treat acute and chronic diseases
• Interpret laboratory and test results
• Prescribe and manage medications and other therapies all with an emphasis on health maintenance and disease prevention

Qualifications:

• Current TX license required
• Two or more years as a Nurse Practitioner
• Excellent communication skills
• Problem-solving skills and ability to deal with a variety of situations
• Hospice or Palliative care experience preferred
• Experience EMR preferred
