The role involves conducting psychosocial assessments, supporting patients and families, and collaborating with multidisciplinary healthcare teams to ensure high-quality care. This position will cover a full-time equivalent leave of absence. Responsibilities Perform detailed psychosocial assessments for patients Develop and implement care plans in coordination with healthcare providers Provide emotional support, counseling, and resource guidance to patients and families Collaborate with interdisciplinary teams to optimize patient outcomes Maintain accurate and timely documentation Requirements Master’s Degree in Social Work (MSW) – required Licensed Master Social Worker (LMSW) in the State of New York (must obtain within 1 year of hire) Strong communication and interpersonal skills Knowledge of psychosocial assessment techniques Experience in a hospital or outpatient medical setting preferred Transplant experience is a plus but not required #J-18808-Ljbffr Clough AMEC.
