Post-Hospital Social Services Navigator

National Health Foundation

National Health Foundation in Arleta, CA seeks a Social Services Coordinator to provide post-hospital care for guests transitioning from acute care. You will manage casework, assess needs, and coordinate housing and services, reporting to Social Services Manager. Requirements include a Bachelor’s degree (Master’s in social work preferred) and three years in healthcare or social services.
