AmeriCorps is seeking a dedicated member to support Perry Family Free Clinic’s mission in Wisconsin. You will connect underserved residents to healthcare through outreach, referrals, and collaboration with social workers and local organizations. You will assist with patient registration, intake, appointment coordination, follow-up calls, reminders, and patient navigation. You will help facilitate community health screenings under staff supervision, support initiatives like Operation Diabetes, #J-18808-Ljbffr AmeriCorps
