This role is designed for a remote work setting, allowing you to provide counseling services from the comfort of your own space while remaining an integral part of our outpatient team located in Plano, Texas. Key Responsibilities\n

• Conduct ongoing assessments of patients’ mental health needs, utilizing evidence-based practices to tailor therapy to individual circumstances.

• Facilitate effective communication among supervisors, direct care staff, family members, and other stakeholders regarding patient progress and care plans.
\n
• Develop and provide a proposed schedule of appointments and home visits for supervision and work management.
\n
• Coordinate and execute in-home visits to ensure comprehensive observations and evaluations are performed, enhancing service effectiveness.
\n
• Monitor the implementation of therapeutic plans, ensuring adherence to established goals and timelines.
\n
• Engage actively in interdisciplinary team meetings and relevant discussions to foster collaborative treatment planning.
\n
• Generate detailed monthly and quarterly progress notes that outline patients' developments and necessary adjustments to treatment plans.
\n
• Organize and deliver training sessions for staff and team members to ensure competence in program plan execution and documentation processes.
\n
• Maintain organized records, making information easily accessible for consultation and management purposes.
\n
• Serve as a liaison with the mental health community and the public, fostering partnerships that support organizational development goals.

• Contribute to the creation of documentation and training materials aimed at continuous improvement of treatment programs.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Hold an active and unrestricted professional counselor license (LPC) in the state of Texas.

• A minimum of two years of counseling experience, particularly in a clinical setting.

• A strong understanding of ethical and legal standards in mental health practice.
\n
• Proficient in the use of electronic medical records (EMR) software and related technologies.
