Welltel is a rapidly growing New York-based telehealth organization providing in-home and virtual care services to patients referred through health plans, care agencies, and community health partners. Our mission is to improve access to quality care while delivering a seamless experience for both patients and providers.

We are seeking an experienced Clinical Director to lead clinical quality, provider performance, compliance, and program development across our expanding provider network. This is a leadership role for a clinician who wants to build, improve, and scale clinical operations while ensuring exceptional patient care and documentation standards.

Position Summary

The Clinical Director will be responsible for establishing and maintaining clinical excellence across the organization. This individual will oversee provider quality, clinical documentation standards, compliance initiatives, onboarding and training programs, and the development of new clinical service lines. The ideal candidate combines strong clinical expertise with leadership experience and a passion for building systems that support both quality care and organizational growth.

Key Responsibilities

Clinical Quality & Documentation

·        Set the documentation standard for every visit type, including HPI, history, medication review, assessment, treatment plan, PCP information, and follow-up justification.

·        Make sure every note supports its medical decision-making and the level billed, under CMS and payer guidance.

·        Build and maintain EMR templates, macros, and structured forms in eClinicalWorks, and shape how AI-assisted documentation tools fit into the workflow.

·        Run an objective QA scorecard with a target audited chart pass rate above 95%.

·        Monitor provider performance and implement quality improvement initiatives.

Compliance & Regulatory Oversight

·        Serve as the internal expert on New York Medicaid and Medicare telehealth rules, including audio-visual versus audio-only documentation and billing requirements.

·        Enforce non-negotiable clinical and compliance gates, including telehealth consent and proper scope-of-practice language.

·        Keep providers in their lane: assess and document, refer back to the PCP or specialist, and avoid managing care they do not own.

·        Work with Welltel’s attorney and FQHC partners to find compliant ways to support operations and patient care, not just to say no.

·        Ensure adherence to clinical documentation, billing, and regulatory standards.

Provider Leadership & Development

·        Own onboarding and training for every new provider before they see patients.

·        Coach providers using real charts and practical feedback.

·        Run performance improvement plans when providers do not meet documentation, compliance, or clinical standards.

·        Terminate providers who do not meet the standard, with a clear escalation path, including immediate termination for patient abandonment or gross negligence.

·        Be the clinical voice providers respect and go to with questions.

·        Foster a culture of accountability, collaboration, and continuous improvement.

Program Development

·        Design the follow-up care model, including when a return visit is justified, what happens during the return visit, and how labs and results are handled.

·        Ensure appropriate coverage and escalation for abnormal results.

·        Build clinical pathways for patients with and without a PCP.

·        Provide clinical oversight for new visit types and programs, including Welltel’s nursing home discharge program.

·        Partner with operational leaders to improve patient outcomes and care delivery processes.

Qualifications

Required

• Active NP, PA, MD, or DO license in good standing.

• Experience diagnosing and treating patients in a clinical setting.

• Demonstrated experience leading clinical quality, documentation, compliance, or provider performance programs.

• Experience supervising and mentoring providers.

• Strong understanding of clinical documentation, quality assurance, and regulatory compliance.

• Experience developing processes, workflows, programs, or clinical initiatives.

• Excellent communication, leadership, and problem-solving skills.

Preferred

• Telehealth experience.

• Knowledge of New York Medicaid and Medicare telehealth regulations.

• Experience in home care, MLTC, FQHC, value-based care, or population health settings.

• Experience with eClinicalWorks (eCW).

• Experience launching or scaling clinical programs.

• Bilingual skills are a plus.
