Serve as a vital resource in providing skilled home health nursing care in a field-based setting throughout the local community. This full-time, 13-week contract offers an opportunity for experienced registered nurses to contribute to patient recovery while enjoying daytime hours and the flexibility of working in patients homes.

Schedule & Hours:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct thorough OASIS assessments and develop personalized care plans.
• Deliver direct nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
• Track patient progress, recognize significant health changes, and communicate promptly with the broader care team and physicians.
• Provide comprehensive education to patients and families about conditions, treatments, and preventive strategies.
• Accurately document all patient encounters and updates using Home Care Home Base (HCHB).
• Work collaboratively with interdisciplinary team membersphysical therapists, occupational therapists, social workers, and aidesto ensure holistic, coordinated care.
• Participate in ongoing staff meetings, case conferences, and professional development activities.
• Uphold all clinical, regulatory, and agency standards during service delivery and documentation.

Required Qualifications:

• Current RN license (in good standing) in the state of practice
• At least one year of recent home health nursing experience, with field-based care preferred
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance
