Case Manager (Bilingual English/Spanish Preferred)
Location: Bronxwood Assisted Living Facility, Bronx, NY

Bronxwood Assisted Living Facility is looking for a compassionate, organized Case Manager to join our team. In this role, you’ll be the person residents and families rely on: coordinating care, guiding admissions, and making sure every resident receives the support they need. You’ll work alongside a collaborative team of healthcare professionals who truly care about the people we serve.

What You’ll Do

• Serve as the main point of contact between residents, families, physicians, and outside care providers

• Conduct resident assessments and coordinate admissions and re-admissions from the community, hospitals, and nursing homes

• Coordinate care across the continuum, working closely with nursing and the interdisciplinary team

• Advocate for residents and uphold the Residents’ Bill of Rights in every interaction

• Maintain accurate, timely documentation in ALIS in line with facility and regulatory standards

What We’re Looking For

• At least 1 year of case management experience (required)

• Long-term care or assisted living experience is a strong plus

• Fluency in English and Spanish (strongly preferred)

• Experience with ALIS or a similar EMR system is a plus

• A clear, warm communicator who builds trust with residents, families, and outside providers

• Highly organized and able to manage multiple priorities

• A genuine passion for working with seniors and a team-first mindset
