Home Health Nurse Practitioner

Position Details:

Certifications: BLS required
Experience: Minimum 1-2 years in acute care, hospital, or home health/hospice settings
Start Date: Within 2-3 weeks after offer (quick feedback response time)
Schedule: Full-time, Four 10 hour shifts, rotating between Mon - Fri, 8:00 AM - 7:00 PM (40 hours/week)
Duties: Travel to patients' homes to provide care, conduct assessments, and complete documentation
Patient Volume: 3-5 patients/day
