LTSS Case Manager — Home-Visit Care Navigator

UnitedHealth Group is seeking a Case Manager in the LTSS program to support home- and facility-based care coordination. You will intake, assess, plan, and monitor services, maintaining confidentiality and ensuring timely documentation across systems. Ideal candidates have a human behavioral science degree or equivalent LTSS experience, plus proficiency with MS Office.
