Sample Supports’ is seeking a Residential Coordinator to support adults with intellectual and developmental disabilities living in host homes, with family caregivers, and in supported apartment settings.

This role combines care coordination, provider oversight, advocacy, compliance, and direct relationship-building. Residential Coordinators maintain an assigned caseload, monitor the quality and safety of services, support residential providers, and coordinate with each individual’s interdisciplinary team.

This position has three levels based on experience, demonstrated competency, and leadership responsibilities. Qualified applicants will be considered for the level that most closely matches their background.

What You’ll Be Doing

• Manage an assigned caseload of adults receiving residential services
• Monitor each individual’s health, safety, rights, preferences, and overall quality of life
• Build consistent, professional relationships with individuals, host home providers, family caregivers, and support teams
• Advocate for person-centered, appropriate, and high-quality services
• Coordinate with families, guardians, case managers, advocates, state personnel, therapists, medical professionals, and other interdisciplinary team members
• Monitor diagnoses, routine and PRN medications, medical appointments, health assessments, protocols, and recommended follow-up
• Develop and implement service, safety, treatment, behavior, and support plans in partnership with leadership and interdisciplinary teams
• Conduct required health, safety, and welfare visits
• Complete annual unannounced home visits and residential compliance inspections
• Supervise, train, support, and hold residential contractors accountable for service and documentation expectations
• Provide education and resources to individuals, families, providers, and support teams
• Participate in team meetings, clinical supervision, court hearings, administrative reviews, Human Rights Committee reviews, and other case-related meetings
• Complete accurate documentation, service tracking, and individual records within 24 hours of service delivery or the required deadline
• Audit individual and provider records for accuracy, completeness, and regulatory compliance
• Follow Sample Supports policies and all applicable HCPF, CDPHE, licensing, and local, state, and federal requirements
• Use sound judgment to resolve complex concerns, make timely decisions, and respond effectively during emergencies
• Participate in the residential program’s 24/7 on-call rotation as scheduled or assigned
• Provide backup direct care, transportation, or shift coverage when needed to maintain continuity of services
• Implement service plans, behavior plans, support plans, medical protocols, and other client-specific requirements
• Use trained NVCI techniques, including approved physical intervention when necessary and permitted to maintain safety
• Immediately report suspected mistreatment, abuse, neglect, or exploitation as a mandatory reporter
• Use email, electronic records, HR systems, and data-tracking applications effectively
• Adapt to changing program, organizational, and regulatory needs
  

Qualifications:

• Bachelor’s degree in human services, human development, social services, psychology, or a related field preferred
• At least three years of residential, case-management, management-level, or intellectual and developmental disability services experience
• Demonstrated success managing complex or high-acuity cases
• Experience mentoring or training employees
• Advanced knowledge of IDD regulations, compliance requirements, and residential program operations
• Ability to support the Residential Manager and serve as a resource for other coordinators
• Ability to complete and maintain any specialized certification required for the position

Requirements for All Levels

• Must be at least 18 years old due to work-related driving responsibilities
• Must maintain a valid driver’s license and reliable transportation
• Must be willing and able to transport individuals receiving services when needed
• Must be able to travel between residential settings, appointments, meetings, and Sample Supports locations
• Must successfully complete and maintain CPR, First Aid, Medication Administration, NVCI, and other required training or certifications
• Must be able to perform the physical responsibilities of the position
• Must be able to respond calmly and professionally during emergencies and behavioral escalations
• Must be able to provide direct care and coverage when program needs require it
• Must complete all required background checks due to working with at-risk adults

Coordinator Levels (I-III)

Residential Coordinator I: Entry-level coordinator for candidates developing experience in residential or IDD services.

• High school diploma or GED required; related bachelor’s degree preferred
• One to two years of relevant experience preferred

Residential Coordinator II: Experienced coordinator who can independently manage a full caseload.

• Related bachelor’s degree preferred
• Two to four years of IDD or similar service experience
• Experience coordinating with interdisciplinary teams
• Trauma-informed care experience preferred

Residential Coordinator III: Advanced coordinator with strong residential, compliance, and leadership experience.

• Related bachelor’s degree preferred
• Three or more years of residential, case-management, leadership, or IDD experience
• Experience managing complex cases and mentoring employees
