Key Responsibilities\n

• Conduct thorough evaluations and assessments of clients enrolled in hospice care, utilizing a trauma-informed approach to understand their unique needs.

• Actively participate in the development and implementation of Individualized Family Service Plans (IFSPs) that address emotional and psychological aspects of care.

• Monitor clients' progress and treatment outcomes by collaborating with interdisciplinary teams, ensuring that necessary adjustments to care plans are made timely.

Qualifications\n

Compensation and Benefits\n
