Key Responsibilities\n

• Conduct in-depth assessments to identify clients' social, emotional, and environmental challenges, utilizing appropriate assessment tools.

• Develop actionable service plans that encompass therapeutic goals, intervention strategies, and progress tracking for individuals and families.

• Provide individual and family counseling, facilitating therapeutic sessions to address issues such as grief, loss, trauma, and mental health disorders.

• Offer crisis intervention and risk assessment to clients in immediate need, formulating safety plans when necessary.

• Collaborate with clients to create personalized service plans and connect them with essential community resources such as food assistance, housing programs, or mental health services.

• Engage in community outreach initiatives to raise awareness about agency services and foster stronger community ties.

Qualifications\n
