Key Responsibilities\n

• Establish and maintain a trusted rapport with clients, serving as a compassionate advocate in their journey toward mental wellness.

• Conduct thorough assessments and screenings using evidence-based practices to identify individual needs and develop targeted treatment plans.

• Implement integrated care plans that facilitate access to necessary services, ensuring effective communication between clients, family members, and other healthcare providers.

• Provide a range of focused prevention, treatment, and recovery services, including individual, group, and family therapy, to address clients’ presenting problems.
\n
• Assist with crisis interventions and facilitate necessary therapeutic approaches during critical moments.
\n
• Monitor clients' progress and treatment outcomes, making adjustments to care plans as needed to enhance recovery trajectories.

• Participate in quality improvement initiatives, working closely with colleagues to elevate service delivery standards.
\n
• Ensure that all documentation and reporting comply with ethical guidelines and organizational standards, including timely submission of all required reports.

• Be available for on-call support during and outside program hours as assigned.

Patient Population / Client Focus\n

This role focuses on clients suffering from major mental illnesses and severe emotional disorders, primarily within the hospice care setting. You'll encounter a diverse group of individuals, including those at the end stage of life who require both emotional and therapeutic support. Your role will be pivotal in providing comfort, emotional stability, and resources to enhance their quality of life.

Qualifications\n

• An active, unrestricted Licensed Clinical Social Worker (LCSW) certification.

Compensation and Benefits\n

This position offers a competitive salary ranging from $70,000 to $85,000 annually, commensurate with experience and qualifications.
