Job Description

Requirements:

• Active RN license in state of practice (or compact license as applicable)
• BSN preferred; ADN accepted per facility policy
• BLS certification required
• Ambulatory Care Nursing certification (AMB-BC) preferred
• 1+ years of ambulatory/outpatient care experience required
• Strong triage, patient education, and care coordination skills

Duties:

• Provide nursing care and support to patients in outpatient clinic, physician office, or ambulatory surgical settings
• Perform patient assessments, triage, and telephone/virtual triage as applicable
• Administer medications, injections, and treatments per physician orders
• Coordinate patient care across appointments, referrals, and follow-up testing
• Educate patients and families on diagnoses, treatment plans, medications, and preventive care
• Assist providers with procedures, exams, and minor in-office interventions
• Manage patient flow to ensure efficient clinic operations and minimal wait times
• Collaborate with physicians, specialists, and interdisciplinary team on care coordination
• Accurately document assessments, interventions, and patient communication in the EMR
• Provide chronic disease management support and care plan follow-up
