HIV Outreach & Care Navigator

The HealthCare Connection Inc.

The HealthCare Connection in Cincinnati, OH is seeking an HIV Outreach & Navigation Specialist to support its HIV programs by identifying, engaging, and linking individuals living with HIV to a comprehensive network of care and support services. These services include Infectious Disease, Dental, Vision, Substance Use Disorder treatment, Behavioral Health, Primary Care, and more. This position conducts Ohio Department of Health CTR HIV testing, provides capacity-building education, and leads #J-18808-Ljbffr The HealthCare Connection Inc.
