Work Environment

This role involves delivering care and therapy services in the patients' residences. As a home-based Physical Therapist, you will have the flexibility to navigate different environments, which can vary widely—from comfortable homes to more challenging living situations. Key Responsibilities

• Conduct comprehensive evaluations to assess each patient's physical capabilities, limitations, and therapy needs through direct observation, interviews, and physical tests.
• Develop and implement tailored physical therapy plans that align with each individual's recovery goals and clinical assessments, ensuring continuity of care.
• Administer diverse hands-on treatment techniques, which may include therapeutic exercises, manual therapy, gait training, and balance enhancement.
• Educate patients and their families on recovery processes, pain management tactics, and at-home exercises designed to promote continual progress between therapy sessions.
• Document all treatment modalities and patient progress meticulously, adhering to regulatory standards and enabling seamless communication across multidisciplinary teams.
• Continuously monitor patient advancements, making timely adjustments to treatment plans while providing necessary updates to referring physicians and family members.

Patient Population / Client Focus

Your patients may include individuals recovering from surgeries, elder adults seeking support in mobility and independence, as well as athletes aiming to regain their physical functionality. The settings of care are diverse, addressing the unique challenges that each patient's home environment may present. You will play a vital role in fostering independence in patients of varying ages, ensuring their path to recovery is as smooth as possible.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• An active and valid Physical Therapy license in the state of New Jersey.
• A minimum of 2 years of practical experience in physical therapy, with a preference for those who have worked in homecare or outpatient settings.
• Strong clinical assessment and manual therapy skills, complemented by a thorough understanding of rehabilitation methodologies.
• Certification in CPR and First Aid is preferred; additional certifications in specialties like geriatrics or neurological rehabilitation are advantageous.
• A valid driver's license and access to reliable transportation are essential for travel to patient homes.

Compensation and Benefits
