Work Environment\n

As a remote employee, you will be part of a vibrant and supportive team dedicated to delivering top-tier hospice services. Our focus on technology and a flexible work culture allows you to balance professional commitments while addressing the needs of our vulnerable clients. Your work hours will primarily be Monday through Friday from 8:00 AM to 4:30 PM, with occasional flexibility required for evening or weekend client needs. On-call availability may also be necessary to provide urgent support.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of seniors, considering emotional, social, and mental health factors impacting their well-being.

• Develop personalized care plans collaboratively with clients and their families that target individual goals, addressing both their interests and needs.

• Provide counseling and emotional support to seniors and their families, helping them effectively manage life transitions, grief, and challenges associated with hospice care.

• Advocate for clients to ensure they have access to essential community resources, services, and rights, promoting their autonomy and dignity.

• Collaborate closely with interdisciplinary teams including healthcare professionals, nurses, and caregivers to ensure a cohesive and comprehensive care approach.

• Conduct regular home visits to monitor clients’ living conditions and overall well-being, addressing any immediate concerns or safety issues.

• Facilitate communication between family members to resolve any conflicts related to the senior client’s care and ensure everyone’s needs are addressed.

• Provide education to clients and families about relevant community resources, support groups, and programs that promote social engagement and enhance quality of life.

• Maintain meticulous documentation of client interactions, assessments, care plans, and progress notes adhering to industry standards and regulations.

• Stay updated on best practices in social work, especially related to hospice care, to consistently comply with ethical standards and organizational policies.

Qualifications\n

• Must hold a Master’s degree in Social Work from an accredited university.

• Possess a current, valid state licensure as a Clinical Social Worker (LCSW).
