Job Description Pinellas and East and West Pasco Counties Seeking a Per Diem Physical Therapists (PT) to join our team in a Home Health setting! This is an exciting opportunity to join an evolving therapy team and deliver exceptional care for patients and families receiving Home Health Services . Conduct patient assessments and evaluations to determine their physical condition, limitations, medical histories, and personal goals. Evaluate patient'sstrength, range of motion, neurological status, balance, and ambulation or wheelchair mobility. Provide direct patient care under the authorization of a physician. Create and update treatment plans, testing,measuring the patient’s strength and flexibility. Confer with physicians, patients, families, nurses, therapists, social workers, and members of thepatientcare team. Develop a treatment plan for patients with specific goals and tasks to assist in meeting functional goals. Evaluate patient’s homeand identify potential improvements based on their needs. Complete all clinical notesusing the company’s EMR system. Contribute to meetings and case conferences to ensure coordinated and comprehensive care plans for patients. Oversee physical therapy assistants and physical therapy aides. Qualifications Current Florida Physical Therapy License with minimum two years of experience.
